I, the Individual, understand that the submission of this application form does not imply its acceptance or agreement to compensation in return for services provided by Individual as Performer, Artist or Presenter; that GeoParadise will not provide any benefits traditionally associated with employment to Individual; and that Individual is responsible for his/her own insurance coverage in the event of personal injury or illness as a result of Individual's services to GeoParadise or occurring while Individual is providing said services.

I understand that, if GeoParadise agrees with the individual to establish a Client-Vendor relationship, a separate professional services agreement will be produced to cover the particular terms of said relationship out of the scope of the present agreement.

I acknowledge Geoparadise reserves the right to terminate this agreement for violation of the terms listed below or for good cause. I agree to comply with any and all rules, regulations, terms and conditions set forth by GeoParadise, including but not limited to the following:

1. WAIVER AND RELEASE: I, the Individual, release and forever discharge and hold harmless GeoParadise and its successors and assigns from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from the services I provide to GeoParadise. I understand and acknowledge that this Release discharges GeoParadise from any liability of claim that I may have against GeoParadise with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services I provide to GeoParadise or occurring while I am providing said services.

2. INSURANCE: Further I understand that GeoParadise does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance of any nature in the event of my injury, illness, death or damage to my property. I expressly waive any such claim for compensation or liability on the part of GeoParadise in the event of such injury or medical expenses incurred by me.

3. MEDICAL TREATMENT: I hereby release and forever discharge GeoParadise from any claim whatsoever which arises or may arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my tenure as performer/artist/presenter with GeoParadise. I consent to have medical treatment that may be deemed advisable in the event of injury, accident and/or illness that may result from the services I provide to GeoParadise or occurring while I am providing said services. I release GeoParadise and all persons participating in any such medical treatment from all responsibility for any such actions.

4. ASSUMPTION OF RISK: I understand that the services I provide to GeoParadise may include inherently dangerous activities due to the nature of the services or due the natural environment where the event takes place. I hereby expressly assume the risk of injury or harm from these activities and Release GeoParadise from all liability for injury, illness, death or property damage resulting from the services I provide as a performer/artist/presenter or occurring while I am providing said services.

5. PHOTOGRAPHIC RELEASE: I grant and convey to GeoParadise all right, title, and interests in any and all photographs, images, video, or audio recordings of me or my likeness or voice made by GeoParadise in connection with my providing services to GeoParadise.

I understand that being accepted as a performer/artist/presenter at Tribal Gathering I am not guaranteed any additional rights or privileges from other attendees. I understand that I am expected to provide the services submitted in this application form (art exhibition, workshops, talks, performances, etc) with harmony and respect for the other participants.

In order to maintain a safe and enjoyable atmosphere for all participants, GeoParadise reserves the right to escort any person off of the premises at any time for any reason. These Terms and Conditions may be changed by GeoParadise at any time and I understand that the updated Terms and Conditions will apply to me.

The Individual will receive a copy of the terms and conditions upon arriving at the gathering location.

6. DANGEROUS ACTIVITY: I agree that I will not engage in inherently dangerous activities, including but not limited to the following: any treatment or therapy involving needles, illegal drugs, osteopathic/chiropractic practices, or any other inherently dangerous practice or treatment.

7. SCOPE OF SERVICE: I agree to refrain from any unauthorized commercial activity or any other activity except for the specific service I have agreed to provide.
